New Insights on Neck Flexibility and Aging



As people move beyond age 40, a gradual loss of neck flexibility is common, but it’s not unavoidable. Research published in Spine sheds light on how aging affects neck function and what that means for maintaining mobility and comfort over time.

Physical therapists have long observed that reduced neck range of motion is associated with discomfort and tends to decline with age. However, detailed data on exactly how this happens has been limited. New findings now provide clearer guidance on what changes to expect and how to respond.

In a large study of over 1,200 healthy volunteers, researchers led by Masaaki Machino, MD, examined how neck mobility changes across different age groups. One key takeaway: the ability to look downward is generally preserved, while the ability to look upward declines with age. This loss of extension can contribute to neck pain and stiffness.

The study also confirmed that noticeable reductions in range of motion often begin around age 40, with significant differences between younger adults and those in middle age. More broadly, research shows that cervical spine motion tends to decrease progressively with age, including measurable declines in extension and overall mobility over time.

From an anatomical standpoint, aging is associated with changes in spinal alignment. The natural forward curve of the neck, known as cervical lordosis, often becomes more pronounced with age, while overall mobility decreases. Researchers suggest this may be linked to factors such as spinal compression, muscle weakness, and shortening of connective tissues.

What This Means for Daily Health

These findings highlight an often-overlooked point: the neck needs regular movement and strengthening just like the rest of the body. While many people stretch their legs or back, the neck is frequently neglected.

Incorporating simple exercises can help maintain mobility:

One particularly effective movement is head retraction (posterior translation). This exercise strengthens neck muscles and improves posture. It can be performed by lying on a surface with the head unsupported and moving the head up and down in a straight, controlled motion, keeping the eyes fixed on a point to avoid rotation.

Age-related changes in neck mobility are normal, but they are not entirely out of your control. With targeted movement and attention to posture, it’s possible to maintain function and reduce discomfort well into later decades.
